Licensed Social Worker

Overview of the Role:

As a Licensed Social Worker, you will play a pivotal role in delivering direct social work care management services to participants of the PACE program. You will provide crucial support through participant screening, case management, counseling, and referral services. Your work will ensure PACE participants and their families receive the necessary resources and guidance to thrive within the community. This role will see you engaging with interdisciplinary teams, advocating for participant needs, and being a key liaison between caregivers, representatives, and community agencies.

Responsibilities:
  • Conduct comprehensive assessments, develop care plans, and participate in periodic re-assessments for PACE participants.
  • Document participant changes and maintain accurate case management records.
  • Communicate effectively with interdisciplinary team members about participant status and needs.
  • Coordinate 24-hour care delivery, offering individual and family counseling.
  • Develop and lead group counseling sessions and organized activities.
  • Refer participants and families to community agencies, acting as a bridge and advocate.
  • Foster team building among staff to ensure cohesive group dynamics.
  • Consult with staff regarding social, emotional, and cultural factors affecting health care.
  • Partake in policy development and quality improvement processes.
  • Perform home visits to participant homes as necessary.
  • Engage actively in team meetings and participant care planning sessions.
Requirements:
  • Master's degree in Social Work from an accredited institution.
  • Preferably holding ACSW or another NASW-recognized certification.
  • Valid state license in social work.
  • Two years of experience in a setting with interdisciplinary teams (hospital, nursing home, or community-based preferred).
  • Preferably one year of experience with frail or elderly populations, with training provided if necessary.
  • Reliable transportation, valid driver's license, and state-required liability auto insurance.
  • Medically cleared for communicable diseases and up-to-date immunizations.
  • Pass a comprehensive background check, including but not limited to Medicare/Medicaid exclusion lists, criminal history, education, and license verification.
